Ascariasis

Man has been “familiar” with the roundworm since ancient times. Mention of this parasite is found in the world-famous Ebert papyrus (VI century BC), in the writings of Hippocrates and Avicenna. The first scientific description of the parasite was given in 1758 by Carl Linnaeus.

The early stages of the disease are accompanied by allergic reactions, the later stages by gastrointestinal disorders; severe complications are possible. The causative agent is the roundworm roundworm (Ascaris lumbricoides). Within 2-3 months, the length of the male reaches 18-25 cm, the female - 25-40 cm. Fertilized oval eggs have a size of 0.04-0.05 mm.

There are so many eggs that they can be easily detected in a smear. In the external environment, eggs develop at temperatures from +10 to +37° C. At temperatures above +37° C they begin to die. Roundworm eggs are very resistant to external influences; Having 5 shells, they can withstand a temperature of -27° C for several days and a temperature of -50° C for a day. Eggs can exist in the soil for 10-13 or even more years.

Everyone should know this! A person can become infected with ascariasis at any time of the year, but more often in the summer this happens when eating vegetables, herbs, fruits, and berries (especially strawberries). The source of infection is dirty hands: the eggs mature to the invasive stage under the nails within 12 days.

Ants, flies, mosquitoes, horseflies and other insects also contribute to the contamination of food products. The eggs are sticky: they stick to shoes and are easily carried indoors, they can be spread by dust, and develop in water with an oxygen content of more than 8-12 mg per 1 liter of water. Large floods increase the possibility of infection with ascariasis.

Wastewater used in farms where there is no sewage system due to the high cost of treatment facilities can play a role in ascariasis infection. Wastewater in ponds and lakes helps maintain soil fertility. Fish (carp) are grown there and there are plants consumed raw.

In the intestine, roundworm larvae emerge from the eggs, migrate through the portal vein to the liver, from the liver into the hepatic veins and then are carried through the bloodstream into the inferior vena cava, and then into the heart. From the heart, migrating larvae enter the artery and capillaries of the lungs, where, with active drilling movements, they tear the walls of the capillaries and penetrate the alveoli, and then into the bronchioles and trachea. Through the trachea, the larvae migrate to the pharynx and enter the oral cavity, from where some of them are removed with saliva and die in the air. The rest are swallowed with saliva and return to the intestines. In the intestines, after 2-3 months they reach sexual maturity. From one to many hundreds of roundworms can parasitize at the same time. Life expectancy is 12-15 months, after which they die and are excreted in the patient’s feces. However, as a result of repeated infections, the infestation can last for years. On average, several hundred million people get sick every year on the planet.

Clinic

. The clinical course of ascariasis is divided into two phases: early (migratory) and late (intestinal). Already 8 hours after infection, the larvae enter the liver and accumulate there maximally on about the fourth day. In the lungs, larvae accumulate gradually: a few penetrate within a day after infection, while the maximum amount accumulates by the end of the seventh day. For 5 days, migrating larvae feed on their own reserves of nutrients, and then on the blood of the host. Depending on the number of parasites, they can “eat” up to 0.5 liters of blood per day from a person, releasing the products of their vital activity in appropriate quantities. In the early phase, “volatile” infiltrates of the lungs appear (characteristic signs: cough, sometimes fever, in the lungs - foci of dry and moist rales, radiologically - darkening 3-4 cm in size; occasionally dry or exudative pleurisy; in the blood - eosinophilia). The duration of this phase is 4-10 days, rarely longer.

In the intestinal phase, dyspeptic disorders, abdominal pain, sometimes diarrhea, moderate anemia, headaches, dizziness, increased mental fatigue occur, and children occasionally experience seizures resembling epilepsy. Possible complications - intestinal obstruction (spastic, obstructive, volvulus, intussusception, nodulation); a characteristic feature of intestinal obstruction is the release of roundworms with vomiting and the ability to palpate a moving “tumor” - a ball of worms; peritonitis as a result of perforation of the intestinal wall by roundworms or their release into the abdominal cavity through surgical sutures placed on the intestinal wall; penetration of helminths into the appendix (ascariasis appendicitis), respiratory organs with the development of asphyxia, into the liver and bile ducts. In this case, compression of the liver ducts most often occurs (obstructive jaundice), and in some cases, liver abscess. Parasites can also migrate to the pancreas, leading to severe pancreatitis.

The most typical symptoms of ascariasis are cough (dry or with mucous sputum), shortness of breath, and possible development of pleurisy. Usually, already on the third day after infection, malaise, lethargy and weakness appear. Sometimes the temperature rises to 38 °C.

What's happening?

Organs such as the liver and spleen increase in size. Lymph nodes may be enlarged. With further development of the disease ascariasis, appetite decreases, abdominal pain and a feeling of nausea appear, and sometimes vomiting. The chair is unstable. The entire course of the disease is accompanied by malaise and weakness. Memory decreases, sleep becomes restless. In young children, infection with roundworms can lead to severe pneumonia, which can be fatal, or the development of intestinal obstruction.

Ascaris parasitism in the body suppresses the immune system and makes the patient more susceptible to infections. There are known complications of ascariasis such as jaundice (hepatitis), acute pancreatitis, purulent cholangitis, and sometimes liver abscess.

Folk remedies for treating parasites have been time-tested, but worms do not give up easily, so taking the same remedy is not enough. Mix clove powder and honey in a 1:1 ratio. Keep a small portion of this mixture in your mouth several times a day and then swallow. Calamus contains the bitter element acorin, which has a healing effect against helminthiasis. To prepare the infusion 1 tbsp. l. crushed calamus rhizomes, pour 1 cup of boiling silicon water, leave in a sealed container in a boiling water bath for 15 minutes, cool at room temperature for 45 minutes, strain.
